OUH Patient Portal

We are currently contacting small groups of patients by email and text to see if they would like to sign up for 'Health for Me'.
We can offer you access to your digital health record via the OUH Patient Portal, which is called 'Health for Me'.
Health for Me is an online system that allows you to easily view parts of your digital health record safely and securely from your computer or smartphone.
You can view results and reports:
- laboratory results
- Haematology
- Biochemistry
- Immunology
- Blood bank
- Microbiology and Virology
- Cellular Pathology
- Radiology.
You can view your health profile:
- allergies
- procedures
- immunisations
- health issues
- medications.
You can view and download clinical correspondence:
- clinic letters
- general letters
- inpatient discharge summaries
- ED discharge summaries
- maternity booking forms
- maternity discharge summaries.
You can also:
- view the date, time and location of upcoming appointments and save them to your local calendar
- receive and reply to messages about your care
- send and receive attachments
- complete health questionnaires online without needing to visit a hospital site.
Currently you can only view information held on our hospital Electronic Patient Record system.
Your GP record is on a different system, but we hope to include this in future.
Only you will have access to your information. You will have your own log in details and security questions.
As Health for Me develops, we will be looking into ways to provide secure access to nominated carers.
OUH patients who have been registered with Health for Me will also be able to access Health Records on iPhone:

If you are interested in getting an account, please ask about Health for Me at your next hospital appointment.
When you are invited to join Health for Me, you will receive an email from Oxfordshire noreply@iqhealth.com from which you will be able to create an account to log in.
