We have an effective document control policy which ensures documents are reviewed regularly and all changes are identified and documented.

Oxford Cell and Tissue Biobank (OCTB) maintains control of the approved versions and will ensure that only the correct version is made available to referring clinicians via this website.

To prevent obsolete documents from unintended use, please contact us for copies of our eligibility criteria, referral forms, adverse event form and death notification form.
