Referral information

The following information is required for a referral to the Oxford Gait Laboratory:

• Patient name and address
  
• Patient date of birth
  
• NHS number
  
• Diagnosis
  
• Name of referrer
  
• Reason for referral (and question to be answered if known)

If you are unsure as to the suitability of a patient for upper limb analysis, please contact us and we will be happy to offer advice.

Please contact Oxford Gait Laboratory along with a clinic letter containing relevant details.
